If you would like to save some time at your first visit, please print and complete these forms prior to arriving.
Also if you need to obtain old medical records prior to your visit, please print, complete, and deliver this form to your previous physician's office or hospital's medical record department. They will either mail or fax those records to us directly. Note that depending on those facilities, this process can take from a week to three months or longer.
